The effect of nursing intervention based on the integrated theory of health behavior change on the health behavior of patients with diabetes retinopathy

Objective To explore the effect of nursing intervention based on the integrated theory of health behavior change(ITHBC) on the health behavior of patients with diabetes retinopathy(DR). Methods 157 patients with diabetes retinopathy admitted from July to December 2022 in a tertiary A hospital in Wuhan,Hubei Province,were selected as the experimental group by convenience sampling method,while 156 patients with diabetes retinopathy hospitalized from January to June 2022 were selected as the control group,and routine nursing was carried out. The disease cognition scale scores,self-management scale scores,and quality of life scale scores were compared between the 2 groups before and after 6 months of intervention. Results After 6 months of intervention,the disease cognition scale scores,self-management scale scores,and quality of life scale scores of both groups improved,and the results in the experimental group were better than these in the control group,and the difference is statistically significant(P<0.05). Conclusion Nursing interventions based on the ITHBC theory can help improve the disease cognition of DR patients,establish and maintain healthy behaviors,improve their self-management level,and improve their quality of life.

Development and evaluation of a risk predictive model for high level of self-reported symptom cluster distress in breast cancer patients undergoing chemotherapy

Objective To develop and evaluate a risk predictive model for high level of self-reported symptom cluster distress in breast cancer patients undergoing chemotherapy. Methods 647 patients who received chemotherapy and met the inclusion and exclusion criteria were selected at 4 tertiary hospitals in Shanghai,Shandong,Jiangsu,and Zhejiang from October 2019 to May 2021. Cases were randomly assigned to a modeling group and a validation group based on 5-fold cross-validation method at a ratio of 8:2. The random forest algorithm was used to develop the risk predictive model in the modeling group. The receiver operating characteristic curve,Hosmer-Lemeshow goodness-of-fit test,calibration curve,and decision curve were used to comprehensively evaluate the prediction performance of the model in the validation group. Risk factors were identified based on the order of the importance of each influencing factors. Results The incidence of high symptom cluster distress was 33.27% in the modeling group and 29.23% in the validation group. The area under the receiver operating characteristic curve of the prediction model was 0.91;the sensitivity was 65.8%;the specificity was 93.5%;Hosmer-Lemeshow goodness-of-fit test was insignificant(P=0.1365);the decision curve was above the reference line. Body image,self-efficacy,and financial burden were the most important predictors. Conclusion The risk predictive model based on random forest algorithm has good predictive performance,which is of great significance to help identify breast cancer subgroups at high risk of symptom cluster distress,and will potentially promote symptom management.

Summary of best evidence for the management of enteral nutrition with gastric retention in critically ill patients

Objective To retrieve,evaluate and summarize the best evidence from home and abroad on the management of enteral nutrition complicating gastric retention in critically ill patients,and to provide a reference basis for clinical nursing staff to manage enteral nutrition complicating gastric retention in critically ill patients. Methods We searched websites and databases,including UpToDate,BMJ Best Practice,National Institute for Health and Clinical Excellence,Guidelines International Network,Registered Nurses Association of Ontario,Scottish Intercollegiate Guidelines Network,JBI,World Health Organization,Yimaitong guide network,Cochrane Library,PubMed,Embase,Web of Science,Sinomed,CNKI,Nutrition Society website,CINAHL(EBSCO),Wanfang Database,and collected relevant clinical decisions,guidelines,evidence summaries,expert consensuses and systematic reviews. 2 researchers independently assessed methodological quality of included papers and extracted data. Results A total of 18 publications were included,including 3 guidelines,4 expert consensuses,4 systematic reviews,5 randomized controlled trials,1 quasi-experimental study,and 1 best evidence summary. 18 pieces of the best evidence were summarized in 5 areas,including identification of gastric retention,management of enteral nutrition,management of monitoring,management of threshold determination,and management of treatment. Conclusion This study provides evidence-based evidence for nursing staff to manage enteral nutrition in critically ill patients with concomitant gastric retention,which can help reduce the incidence of gastric retention in critically ill patients.

Formulation and application of cardiac rehabilitation grading nursing program in patients with severe coronary artery disease

Objective To develop and implement a grading program of cardiac rehabilitation(CR)nursing intervention among the patients with severe coronary artery disease. Methods In July 2019,we started a specialized group to analyze the characteristics and difficulties of CR nursing among the patients with severe coronary artery disease through literature review and clinical practice. We formulated a program of cardiac rehabilitation grading nursing care and carried out it for clinical application and data collection during August 2019 to January 2021. Results Since the launch of CR grading care,a total of 817 patients with severe coronary artery disease have been included;519(63.5%)patients had underground activities before being discharged,and 13(1.6%)patients had adverse events. Conclusion The program can effectively provide the suitable CR nursing and satisfy the rehabilitation needs among patients with severe coronary artery disease.

Objective To construct the evaluation index of clinical nursing path for elderly patients with femoral neck fracture under Diagnosis-Intervention Packet(DIP) payment,to provide the path form for the clinical nursing work of such patients,and to provide the basis for evaluating their nursing quality. Methods Based on the literature evidence-based and brainstorming methods,the path form was constructed,and the clinical nursing path of elderly patients with femoral neck fracture was initially constructed based on DIP payment. From January to March 2022,17 experts were interviewed by Delphi method for 2 rounds of expert correspondence,and the operation form of their clinical nursing path indicators was established. Results The effective questionnaire recovery rates of the 2 rounds of expert correspondence were 100%;the expert authority coefficients were 0.879,0.918;the Kendall harmony coefficient was 0.476,0.299. The final path indicators included 6 first-class indicators,18 second-class indicators,and 107 third-class indicators. Conclusion The index of clinical nursing pathway for elderly patients with femoral neck fracture based on DIP payment has high scientificity,reliability and practicality,and it can be used as the basis for clinical implementation and evaluation of such patients.

Effects of acupoint application therapy based on midnight-noon ebb-flow for gastrointestinal function in patients undergoing gynecological laparoscopy

Objective To investigate effects of midnight-noon ebb-flow combined with acupoint application on gastrointestinal function of patients after gynecological laparoscopy.Methods 108 patients after laparoscopy in Guangzhou from August 2017 to December 2018 were selected as research subjects who were randomly allocated into group A(midnight-noon ebb-flow combined with acupoint application therapy),group B(rhubarb Shenque acupoint application therapy without time control) and group C (midnight-noon ebb-flow combined with placebo). In addition to the above intervention,all patients received routine nursing care during the study. The gastrointestinal function,including the time of first defecation,the time of first passage of flatus(“gas-out time”) and the recovery time of bowel sound,as well as adverse events,were measured daily.Results The recovery of gastrointestinal function among 3 groups were statistically significant after treatment(P<0.05). The time of first defecation,the time of first passage of flatus and the recovery time of bowel sound in Group A were less than those in the other 2 groups(P<0.05). The time of first defecation and first passage of flatus in Group B was less than that in Group C(P<0.05). But there was no significant difference in recovery time of bowel sounds between Group B and Group C. Meanwhile,Group A ranked with the least adverse events(abdominal distension,nausea,vomiting,abdominal pain and etc.),followed by Group B.Conclusion Acupoint application therapy based on midnight-noon ebb-flow can effectively promote the recovery of gastrointestinal function and reduce the adverse events related to gynecological laparoscopy.
